Generic Name: Penicillamine

Depen may cause serious side effects. Patients using Depen should remain under the close supervision of their doctor. Tell your doctor immediately if you develop a rash, difficulty breathing or wheezing, chills, fever, sore throat, bruising, or bleeding.





Depen is used for:

Treating Wilson disease (excess copper in the body), severe rheumatoid arthritis that has not responded to other treatments, and cystinuria (excess amino acids in the urine, which causes kidney stones). It may also be used for other conditions as determined by your doctor.


Depen is a chelating agent. It works by removing excess copper in patients with Wilson disease, reducing excess cystine in patients with cystinuria, and reducing disease activity in patients with rheumatoid arthritis.


Do NOT use Depen if:


  • you are allergic to any ingredient in Depen or to penicillin

  • you are pregnant or are breast-feeding

  • you have a history of aplastic anemia or agranulocytosis (decreased number of white blood cells) due to penicillamine use

  • you have a history of kidney problems (rheumatoid arthritis patients only), blood in the urine, or pemphigus (blistering skin disease)

  • you are taking gold therapy (eg, gold salts), antimalarial or cytotoxic medicines (eg, quinine, carboplatin), oxyphenbutazone, or phenylbutazone

Contact your doctor or health care provider right away if any of these apply to you.



Before using Depen:


Some medical conditions may interact with Depen. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are planning to become pregnant

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you are scheduled for surgery

  • if you have Goodpasture syndrome, myasthenia gravis, aplastic anemia, or decreased blood platelets or white blood cells

Some MEDICINES MAY INTERACT with Depen. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Antimalarial medicines (eg, atovaquone), cytotoxic medicines (eg, carboplatin, quinine), gold salts, oxyphenbutazone, or phenylbutazone because the risk of side effects, such as kidney problems and blood disorders, may be increased

  • Digoxin because its effectiveness may be decreased

This may not be a complete list of all interactions that may occur. Ask your health care provider if Depen may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.


How to use Depen:


Use Depen as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Take Depen on an empty stomach (1 hour before meals or 2 hours after meals).

  • Take Depen at least 2 hours before or after other medicines containing aluminum or magnesium (antacids), iron, or vitamins that contain iron.

  • If you are taking Depen for cystinuria, drink plenty of water while taking Depen. Drink a pint (16 ounces [480 mL]) at bedtime and another pint during the night, unless directed otherwise by your doctor.

  • It may take 1 to 3 months for Depen to work.

  • If you miss a dose of Depen, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Depen.



Important safety information:


  • Your doctor may prescribe a vitamin B supplement called pyridoxine while you are taking Depen. Follow your doctor's instructions for taking this supplement.

  • Before you have any medical or dental treatments, emergency care, or surgery, tell the doctor or dentist that you are using Depen.

  • LAB TESTS, including complete blood cell counts, platelet counts, liver and kidney function tests, urinalysis, and body temperature, may be required to monitor your progress or to check for side effects. Be sure to keep all doctor and lab appointments.

  • Use Depen with caution in the ELDERLY because they may be more sensitive to its effects.

  • Depen is not recommended for use in CHILDREN with juvenile rheumatoid arthritis. Safety and effectiveness have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: Depen has been shown to cause harm to the fetus. If you become, discuss with your doctor the benefits and risks of using Depen during pregnancy. It is unknown if Depen is excreted in breast milk. Do not breast-feed while taking Depen.


Possible side effects of Depen:


All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Diarrhea; loss of appetite; mild stomach pain; nausea; vomiting.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); black, tarry stools; bleeding or bruising; blood in the urine; burning, itching, peeling, or redness of skin; changes in taste; chills; cough; dark urine; difficult urination; fever; general feeling of discomfort; joint pain; muscle weakness; severe stomach pain; shortness of breath; skin lesions; sore throat; swelling of the feet or legs; vision problems; weight gain; wheezing.

If OVERDOSE is suspected:


Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately.


Proper storage of Depen:

Store Depen at room temperature, 77 degrees F (25 degrees C). Store away from heat, moisture, and light. Do not store in the bathroom. Keep Depen out of the reach of children and away from pets.

Botulinum Toxin Type B


Pronunciation: RYE-ma-BOT-ue-li-num-TOX-in-B
Generic Name: RimabotulinumtoxinB
Brand Name: Myobloc

The effects of Botulinum Toxin Type B may spread from the injection site to other areas of the body, causing symptoms of a serious condition called botulism. These symptoms have been reported hours to weeks after patients have received Botulinum Toxin Type B. Symptoms may include loss of strength; muscle weakness; double or blurred vision; drooping eyelids; hoarseness; loss of voice; loss of bladder control; or trouble speaking, breathing, or swallowing. The risk may be greater in children being treated for spasticity (muscle spasms), but these symptoms may also occur in adults. Contact your doctor right away if these symptoms occur.


Severe and sometimes fatal breathing or swallowing problems have been reported. The risk may be greater in patients who already have breathing or swallowing problems. Tell your doctor if you have or are at risk of developing breathing or swallowing problems.





Botulinum Toxin Type B is used for:

Reducing the severity of abnormal head position and neck pain associated with a certain neck problem (cervical dystonia). It may also be used for other conditions as determined by your doctor.


Botulinum Toxin Type B is a neurotoxin. It works by blocking nerve impulses to the muscles, temporarily paralyzing the muscle.


Do NOT use Botulinum Toxin Type B if:


  • you are allergic to any ingredient in Botulinum Toxin Type B or another botulinum toxin product

  • you have an infection at the injection site

Contact your doctor or health care provider right away if any of these apply to you.



Before using Botulinum Toxin Type B:


Some medical conditions may interact with Botulinum Toxin Type B. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you have nerve problems or diseases (eg, amyotrophic lateral sclerosis, motor neuropathy), muscle problems or diseases (eg, myasthenia gravis, Lambert-Eaton syndrome), bleeding disorders, seizures, or severe weakness or wasting of the muscles at the injection site

  • if you have a history of heart problems (eg, irregular heartbeat, a recent heart attack); swallowing problems; breathing problems (eg, asthma); eye problems (eg, glaucoma); or an overactive thyroid

  • if you have a skin infection, or inflammation, scarring, or other skin disorders at the injection site

  • if you plan to have surgery

  • if you have received Botulinum Toxin Type B or any other botulinum toxin product in the past, especially within the past 4 months

  • if you have never received Botulinum Toxin Type B or any other botulinum toxin product

  • if you have recently received an antibiotic by injection, or you also take muscle relaxants, allergy or cold medicine, or sleep medicine

Some MEDICINES MAY INTERACT with Botulinum Toxin Type B. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Aminoglycosides (eg, gentamicin), anticholinesterase medicines (eg, neostigmine), lincosamides (eg, clindamycin), magnesium, neuromuscular blockers (eg, atracurium), polymyxin, or quinidine because the risk of toxic effects may be increased

  • Anticoagulants (eg, warfarin) because the risk of bleeding at the injection site may be increased

How to use Botulinum Toxin Type B:


Use Botulinum Toxin Type B as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Botulinum Toxin Type B comes with an extra patient information sheet called a Medication Guide. Read it carefully. Read it again each time you get Botulinum Toxin Type B refilled.

  • Botulinum Toxin Type B is usually given as an injection at your doctor's office, hospital, or clinic.

  • If you miss a dose of Botulinum Toxin Type B, contact your doctor right away.

Ask your health care provider any questions you may have about how to use Botulinum Toxin Type B.



Important safety information:


  • Botulinum Toxin Type B may cause dizziness or drowsiness. These effects may be worse if you take it with alcohol or certain medicines. Use Botulinum Toxin Type B with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it. If loss of strength, muscle weakness, or vision problems occur, avoid driving or performing other possibly unsafe tasks.

  • Do NOT take more than the recommended dose or use for longer than prescribed without checking with your doctor.

  • Do not switch brands of Botulinum Toxin Type B. Contact your doctor if you have any questions.

  • Botulinum Toxin Type B contains albumin, which comes from human blood. There is a very rare risk of getting a viral disease or a central nervous system disease called Creutzfeldt-Jakob disease from products with albumin. No cases of these problems have been found in patients who have used Botulinum Toxin Type B.

  • To prevent injury, resume normal activities gradually after using Botulinum Toxin Type B.

  • Botulinum Toxin Type B should not be used in CHILDREN; safety and effectiveness in children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: It is not known if Botulinum Toxin Type B can cause harm to the fetus. If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of using Botulinum Toxin Type B while you are pregnant. It is not known if Botulinum Toxin Type B is found in breast milk. If you are or will be breast-feeding while you use Botulinum Toxin Type B, check with your doctor. Discuss any possible risks to your baby.


Possible side effects of Botulinum Toxin Type B:


All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Anxiety; back pain; dizziness; drowsiness; dry eyes; dry mouth; flu-like symptoms; headache; increased cough; indigestion; nausea; neck pain; pain, redness, swelling, or tenderness at the injection site; runny nose; sensitivity to light; sweating; upset stomach; weakness of the muscles at or near the injection site.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bleeding at the injection site; chest pain; difficulty swallowing or breathing; double or blurred vision, or other vision changes; drooping eyelid; fever, chills, or persistent sore throat; irregular heartbeat; loss of bladder control; loss of strength; paralysis; pneumonia; seizures; severe or persistent muscle weakness or dizziness; shortness or breath; speech changes or problems; swelling of the hands or feet; vaginal irritation, odor, or discharge; wheezing.

Bentyl

What is Bentyl (dicyclomine)?

Dicyclomine relieves spasms of the muscles in the stomach and intestines.


Dicyclomine is used to treat functional bowel or irritable bowel syndrome.


Dicyclomine may also be used for purposes not listed in this medication guide.


What is the most important information I should know about Bentyl (dicyclomine)?


This medication may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert. Drinking alcohol can increase certain side effects of dicyclomine.

Avoid becoming overheated or dehydrated during exercise and in hot weather. Dicyclomine can decrease your sweating, which can lead to heat stroke in a hot environment.


Stop using dicyclomine and call your doctor right away if you have serious side effects such as confusion, hallucinations, unusual thoughts or behavior, fast or uneven heart rate, or if you urinate less than usual or not at all. There are many other medicines that can interact with dicyclomine. Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor. Keep a list with you of all the medicines you use and show this list to any doctor or other healthcare provider who treats you.

What should I discuss with my healthcare provider before taking Bentyl (dicyclomine)?


You should not take this medication if you are allergic to dicyclomine, or if you have:

  • problems with urination;




  • a bowel obstruction or severe constipation;




  • severe ulcerative colitis or toxic megacolon;




  • gastroesophageal reflux disease (GERD);




  • a serious heart condition or active bleeding;




  • glaucoma;




  • myasthenia gravis; or




  • if you are breast-feeding a baby.



To make sure you can safely take dicyclomine, tell your doctor if you have any of these other conditions:



  • ulcerative colitis;




  • an ileostomy or colostomy;




  • a nerve problem (such as numbness or tingling);




  • liver or kidney disease;




  • heart disease, congestive heart failure, high blood pressure, or a heart rhythm disorder;




  • hiatal hernia; or




  • an enlarged prostate.




FDA pregnancy category B. This medication is not expected to be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. Dicyclomine can pass into breast milk and can cause breathing problems or other life-threatening side effects in infants younger than 6 months of age. Do not breast feed a baby while taking this medication. Older adults may be more likely to have side effects from this medicine. Dicyclomine should not be given to a child younger than 6 months old.

How should I take Bentyl (dicyclomine)?


Take exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Follow the directions on your prescription label.


Dicyclomine is usually taken 4 times each day. Your doctor may occasionally change your dose to make sure you get the best results.


Take this medicine with a full glass of water.

Measure liquid medicine with a special dose-measuring spoon or cup, not a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one.


Talk with your doctor if your symptoms do not improve after 2 weeks of treatment. Store at room temperature away from moisture and heat.

What happens if I miss a dose?


Take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

Overdose symptoms may include nausea, vomiting, dilated pupils, weakness or loss of movement in any part of your body, trouble swallowing, fainting, or seizure (convulsions).


What should I avoid while taking Bentyl (dicyclomine)?


This medication may cause blurred vision and may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert and able to see clearly.

Avoid becoming overheated or dehydrated during exercise and in hot weather. Dicyclomine can cause decreased sweating, which can lead to heat stroke in a hot environment.


Drinking alcohol can increase certain side effects of dicyclomine.

Avoid using antacids without your doctor's advice. Use only the type of antacid your doctor recommends. Some antacids can make it harder for your body to absorb dicyclomine.


Bentyl (dicyclomine) side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using dicyclomine and call your doctor at once if you have a serious side effect such as:

  • severe constipation, bloating, or stomach pain;




  • worsening of diarrhea or other irritable bowel symptoms;




  • feeling very thirsty or hot, being unable to urinate, heavy sweating, or hot and dry skin




  • confusion, hallucinations, unusual thoughts or behavior; or




  • pounding heartbeats or fluttering in your chest.



Less serious side effects may include:



  • drowsiness, dizziness, weakness, nervousness;




  • blurred vision;




  • dry mouth, stuffy nose; or




  • mild constipation.

Before using dicyclomine, tell your doctor if you regularly use other medicines that make you sleepy (such as cold or allergy medicine, sedatives, narcotic pain medicine, sleeping pills, muscle relaxers, and medicine for seizures, depression, or anxiety). They can add to sleepiness caused by dicyclomine.

Tell your doctor about all other medicines you use, especially:



  • amantadine (Symmetrel);




  • digoxin (Lanoxin, Lanoxicaps);




  • metoclopramide (Reglan);




  • atropine (Atreza, Sal-Tropine), belladonna (Donnatal, and others), benztropine (Cogentin), dimenhydrinate (Dramamine), methscopolamine (Pamine), or scopolamine (Transderm Scop);




  • bronchodilators such as ipratropium (Atrovent) or tiotropium (Spiriva);




  • bladder or urinary medications such as darifenacin (Enablex), flavoxate (Urispas), oxybutynin (Ditropan, Oxytrol), tolterodine (Detrol), or solifenacin (Vesicare);




  • a heart rhythm medication such as quinidine (Quin-G), procainamide (Procan, Pronestyl), disopyramide (Norpace), flecaininde (Tambocor), mexiletine (Mexitil), propafenone, (Rythmol), and others;




  • irritable bowel medications such as hyoscyamine (Hyomax) or propantheline (Pro Banthine);




  • an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate);




  • nitrate medication, such as nitroglycerin (Nitro Dur, Nitrolingual, Nitrostat, Transderm Nitro, and others), isosorbide dinitrate (Dilatrate, Isordil, Isochron), or isosorbide mononitrate (Imdur, ISMO, Monoket);




  • phenothiazines such as chlorpromazine (Thorazine), fluphenazine (Permitil, Prolixin), perphenazine (Trilafon), prochlorperazine (Compazine, Compro), promethazine (Pentazine, Phenergan, Anergan, Antinaus), thioridazine (Mellaril), or trifluoperazine (Stelazine);




  • steroid medication such as prednisone and others; or




  • ulcer medication such as glycopyrrolate (Robinul) or mepenzolate (Cantil).
